MDR appears to be paying 40% of US revenue straight back to US pharmacies for access to their data. (this is my guess and i'm no finance expert). The company could be much more explicit about this in their communication with investors. There should be a line in their accounts titled "Pharmacy Abatements".
I think this is why their topline looks great and their bottom line is always disappointing.
