All cancer treatment is about balancing efficacy verses side effects. It is very easy to cure a patient of cancer - just dose them high enough that they die. What is hard is keeping the patient alive while you kill the cancer.
While there is resistance to radiotherapies, but the bigger issue is getting the isotopes to the tumour and not having it floating around the body causing side effects.
To answer your question most radiotherapy treatments are typically dosed up the MTD.
