Fatality occurred in January 2020 when the mine was owned by the Pybar guys. CYL didn't acquire the mine until January 2021. As part of the acquisition Pybar agreed to indemnify CYL for any costs incurred from penalties relating to the death. There was cash and shares withheld from the settlement to cover any potential costs that won't be released until the investigation has been resolved.
