OGX had an issue with assay results needing to be corrected in December last year - in that case it was an underreported rather than over reported grade - and wasn't an issue with the assays but with the weighted averaging over the interval - but does highlight that these sorts of issues can occur.

RXL recently announced wide visual mineralisation then had to re-neg on it blaming it on contamination of the core during drilling. (that was RC not diamond).
