As per what I understood from the instructions is once you have paid through BPAY, it automatically computes how many shares you are applying for in addition to the full entitlement and you don't have to send the form back.

I have applied for additional shares under the shortfall offer as there will be a significant available shortfall shares due to ineligible SH as not having an OZ/NZ address.
