All I can think of is that they have assessed CDU operations as high risk (likelihood of a serious accident or incident is high) due to ineffective or nonexistent policies and procedures and the fact that staff are either unaware of their existence or are not trained to enact and enforce these policies and procedures.

There's been an exodus of (professional) environmental and safety staff since late 2015 too, and replaced with under qualified "yes men".

The next move from a regulator will possibly relate to the tailings dam and EPA breeches.

No one in Environment or OHS in their right mind would associate themselves with CDU.
