The shutdown in feb last yesr was with respect to maintenance who didnt have adequate split pin procedures. But you are probably referring to a few breaches last year resulting in short shuts ftom individuals failing to isolate equipment correctly.

The safety manager they have now is actually quite good and has made a few inroads.

The problem is not in the processas or the saftey plans... it is in the application and culture from the process managment team.

The story behind the breaches and subsequent standing down of staff temporarily is an amusing one as told to me. I had a quick chuckle.
