I am pretty confident that PYC will go for a 4th Cohort, as the 75 microgram dose has the potential to be quite strategic and that is why PYC want to do it.
Shri commented back in September that PYC would 'fight' for a 100 microgram dose, however more recently Rohan indicated that PYC have their eyes on a 4th cohort at 75 micrograms.
IMO PYC have being weighing up what sort of readouts such a cohort may deliver and balancing that against the FDA's appetite to provide the green light to go ahead, at an appropriate time. I expect that the appropriate time for such discussions would not precede the Safety Committee's approval of the 3rd cohort.
So in short, I think it will probably happen however it may be a little further down the track. If it works the way we wish, it will be a good thing for PYC and all concerned.
