Yes I have the same form but with letterhead Direct Shares . Rang them and although shows my holdings, they say it will disappear tomorrow and can not transfer the holdings. I have 2 holdings and was going to transfer the larger holding to the small holding name to claim the loss on one. Have you rang CMC to see if you can do this now it is delisted?
