The tax form is the W8BEN which Computershare originally sent in a A4 size envelope which contained a package of information including the DRS / Holder Account Number which could the be used to verify your registration with Computer Share US. Once verified it automatically generated an access code taking another 30 days to arrive. I don't know if you got that pack as most people have now recieved their access code and another W8BEN to fill out as their first one had not been processed . The critical document is the DRS / Holder Account Number to get things moving. If you have not received that then you would need to follow up with Computershare US. I assume that you had greater than $ 500 Au of share at 4.1 cents at the close of trading on the 17th of November and have not had the shares sold off as a smaal parcel default.
