I think the form was with the scheme docs letter. I recall seeing it (or i might be confused given i get quite alot of letters.. I'm just an old bloke refusing to go all electronic haha). My broker usually auto updates bank account for EFT details but I have seen some takeovers where we supposed to send separate request.
