You don’t have to treat at the MTD level, you just need to be able to show why you have chosen the RP2D. It is totally fine to choose to use a lower dose to obtain lower side-effects, you just need to know what this trade-off involves in regards tumour response and side-effects rates. Let’s imagine that at 12GBq the tumour response rate is 60%, but at 16GBq the response rate is 75%, but with double the number of serious side-effects. You can certainly choose to use the 12GBq dose, but you need to know what you are giving up.
