Sometimes matter of opinion. Often claims a fabricated to make claims.

Could be as simple as taking measurements of existing parts taking too long which contractor did not allow enough time for and labor costs.

Often projects go wrong because of incompetence due to lack of skills of project people, poor planning and resoucing and underpricing.

This is made up from dubious claims or gouging on variations directed by the client.

It's like when you have a metricon home and want different tiles. It will cost a heap more than you expect. Variations by client are money for jam.

These contracts allow no variations, so there is only one way your contract profit can go, down, if you had any profit to start with.
Potentially saved by passing these pressures onto your subcontractors or employees.
