No mate you didn’t need to apply to have a pay out and your situation is unique due to the dividend method changing. Normally the payment method whether through extra shares or cash payments remains the same unless you elect to make changes. I recently re bought into FMG and the default option was cash payment. I’m sure you are smart enough to know what to do next however allow me to say if it was me I would contact FMG direct as you are not getting any joy from your super fund. All I can think is that your fund manager has changed the dividend payment method without informing you.
