Cold tumors, at a high level,
are generally tumors with low mutational change. The term cold usually applies to the observation the immune system is not recognizing / stimulated by the tumor as foreign and thus the immune response is poor. There is a relationship to the very common ,almost u Iquitos in older people,
Carcinoma-in-situ (erroneously described sometimes as premalignant but it is a bit more complicated and is a result of a stalemate between the tumor microenvironment and the cancer).
This is exactly the situation where CPIs fail (and why 50% of cancer types are not suitable for CPI therapy) and this where T cell therapy is required.
By stimulating the production of tumor specific CD8 + cells in the middle of the tumor oncolytic viruses effectively turn the tumor from cold to hot. Don’t be confused by the oncolytic part of the name. These viruses mainly work by stimulating an immune response.
Others are trying to insert changes into the canxer cells that then are expressed onto the outside and are recognized by naive immune response or by mamufactured T cell clones (several Car-t companies including one in Australia).
