I just got an email reply to this query from Neville Bassett (AVL company secretary).
We used to be able to get the exercise of options forms directly from AVL (I got them from AVL a few year's back for an earlier batch) but that seems to have changed now.
Neville told me that the Notice of Exercise of Option forms are managed by AVL's share registry - Automic Registry Services, that we need to contact them and that they will send a prepopulated form.
