I think it's very important to take medication under medical supervision, which is why I recommend a sufferer does their research (or a relative can do it) and takes it to their primary care physician. Back pain is a difficult one because there are so many different causes, sometimes physical, sometimes inflammatory. A friend of mine had successful surgery, which completely cured his back pain. From what you've written, it sounds like there could be a physical/mechanical reason. What did the surgeon say?
The drug is Minocycline. Rheumatologist Dr. Brown's work was publicised by Henry Scammell. There was a grass roots movement from patient advocates, which is why the clinical trials eventually went ahead. Only mild cases, tho were allowed to be enrolled, which is why the perception is that this therapy "only works for mild cases". Dr Brown also used it for diffuse scleroderma. You'll see a review from a rheumatologist (gave his real name initially but later changed to a pseudonym likely because of the flak doctors get for thinking outside the box), who said he had a patient with diffuse sclerorderma on this therapy and the condition was reversing. He wrote, "Not bad for an incurable disease".
I was actually very surprised the young man's back pain was relieved by a drug used off-label for inflammatory arthritis (Mainstream rheumatologists do use it but in many cases after it's too late and there's extensive damage to joints) It's interesting that an antibiotic worked so well for him (initially) because I read about a clinical trial in Augmentin in chronic back pain published in a peer-review journal. The theory was that there's initial damage and then bacteria take hold somehow (I can't recall the explanation exactly but it sounded plausible) Several participants gave up tho due to gastro effects. Antibiotics are not entirely benign and you have to keep the microbiome in mind. One point of note, tho, if bugs are an underlying cause or major contributor, MSCs (and likely also MPCs) have a positive effect on bacteria and endotoxins, as Caplan explains in his lecture available on YT.
Also, if nothing is working for you, I suggest looking into low-dose Naltrexone (Prof Angus Dalgleish uses this for some of his melanoma patients, really knows his stuff because he understands its MOA and why a low dose is more effective and that it should be combined with alphalipoic acid (ALA). I also found a small study in late-stage pancreatic cancer. Our family GP prescribed this for my daughter because she was beside herself with red skin syndrome. LDN has also been used in chronic pain management, including back pain. There aren't many large studies, though, because there isn't much profit in cheap drugs.
I do think it's important, tho, to try to get work out the underlying cause of pain or any symptoms and try to address that. My daughter and I had to turn into the Dr House team (she was House and I was Chase) and she literally had a whiteboard and we went through crossing things off.
