I just heard part of the General's statement on TV news - I am puzzled these happenings would not have been "reported up the chain of command"

(BTW - is it impossible in 2020 to have a mic to pass around so all questions can be heard clear.)

I have a question re Army admin. for anyone familiar with Army paperwork.
At what level do people have to submit regular written reports - "..up the chain of command"

For example - do Serjeants or Corporals do a routine written report to anybody on daily happenings?
I assume every combat team would have a junior officer along - would that be right?
Do "1 and 2 Pippers" do a routine written report to a Captain or Major?

    Thanks.
