Not so at all. Even in the third world there are voluntary organisations doing valuable community work with the poor helping the poor. As to how one is treated by the state that varies from country to country and is more of a social/cultural thing than a socialist/capitalist thing.
You seem to have this fixed idea in your head that countries are either one or the other. They are not and almost all are shades in between those economic systems because as I keep repeating socialism and capitalism are both duds if followed too closely.
