I doubt that explains it. Anyone who suffers from inflammatory acne, like cystic acne, is typically very aware (perhaps too much) of their own acne management and skincare regimes. I don't see how an observation effect would suddenly cause them to ramp up their efforts - their efforts are likely already high to begin with.
