A new drug for a skin disease doesn't have to be the best on the market to sell well. If i had the choice between rubbing a man-made chemical soup on my face or trying a product based on a natural chemical with fewer side effects then i would buy the latter despite it having potentially less or similar efficacy.
Also, even if a drug on average has less efficacy than an alternative, it may still have better efficacy ON SOME PEOPLE. So for example if a BOT acne treatment is measured as having less efficacy then an existing chemical soup then it is still worth a try because for some people it will be better than the soup both in terms of efficacy and fewer side effects.
The market isn't just made up of the "average best". There is room for alternatives because everyone reacts different to the medication and to the side-effects.
A BOT treatment for acne or psoriasis will be massive in the market due to these reasons even if it doesn't happen to be the best. (but we are expecting it will be the best!!)
