I only know of one clear cut example, and I reported it to ASIC complete with advice on my opinion as to the weakest link. The same crowd turns up to spread sweetness and light (sarc) at critical points in BOT's history. Note also there are real and possible indications that security has been tightened.
Generally accusations of a leaky ship require proof or at least the strong smell of it so:
if you have suspicions or anything substantive tell ASIC.
