Doctors who want to take on a speciality are forced to work one or two years 'unaccredited' meaning the hours are not counted towards that specialist training. They are paid a base rate though taking on the responsibility of a trained specialist. They are often forced to work long hours, too afraid to complain in case they are not accepted into the specialist training when they apply. Working 12 hour shifts day after day is not uncommon. This is the real world.
To get into a specialist training program, one must accumulate points. Doing courses, research etc is part of that point system. The doctor on that base salary needs to pay for those courses themselves, often thousands of dollars. The wage is not great, the costs to complete courses is high, but overtime helps make ends meet. This is the real world.
