The 100 yr old Mantoux or skin test is still the default LTBI test.
But it seems a change is SLOWLY taking place.
The INERTIA of 100 yrs is hard to change.
The CDC guidlines are due to be changed. It is hoped that the IGRAS get a higher profile than they currently have.
The NICE gudelines in the UK largely reflected the US position.
But there seems to be change there ( my source the CST blog). In a recent TB scare in a UK high school the IGRAS were placed on an EQUAL footing to TST.
The default position was :
(1) Test for LTBI with TST
(2) Because a TST result was NOT REALLY TRUSTED, test the positives with an IGRA
(3) treat the confirmed positives
