I don't think so. I've heard of doctors/researchers dosing themselves.
And for drugs with a high degree of efficacy data in conditions with no alternative options for treatment, patients may be able to access non-approved drugs through special access schemes (at pre-determined dose rates).
But your scenario would likely end in a trial being suspended by the ethics committee. As I understand it (not an expert in clinical trials).
