@robertch. Pinching pennies at the start and paying pounds as a consequence. Lessons to be learnt from Red5. Don't be arrogant. Don't scrimp on pre-mining assessments, pay to operate at thresholds plus a healthy margin (20%), and pay extra for contractors with tier 1 performance.
IMO, holders might be best off throwing $20m at this to fix it, if they front up with a viable plan. Sorry if that sounds like downramping. Short term pain for long term gain.
