If you don't have your Holder ID number (HIN) or SRN, I would go ahead and complete the forms as best you can and attach any evidence you have to support your claim to being a shareholder, post 2010.
In the email I received it specifically states...
It is important that you provide us with the correct name of the registered holder of the Kagara shares and the HIN or SRN relating to the shares (if known).
So if you don't know or have these details, just add what you have and make sure to use the correct name registered for any shares. Wife etc.
You could also contact your CHESS sponsor in the interim period to see if you could get original records and send them off as soon as you can as evidence for your claim. E.g. contact ANZ Margin Services, E*Trade, or CommSec etc...or whoever you bought the shares from.
