I am all for taking a stand but,
Where and who does the information we are giving go to?
After looking at the form I am not quite sure I am happy to give so many personal details to an online form that could be going to anyone.
An expression of Interest should really just be how many shares/Options you hold name and email all the rest should be requested by the lawyer I would think.
I am appreciative of what you are doing but wary of the amount of online scammers these days.
