Provide as much factual information as you can, based on ASX company announcements, not hearsay. If you purchased shares based on the earlier information provided by the Company detail those purchases too. Have your HIN handy as the form asks for it and I believe it gives greater credence to the complaint.
I have had an acknowlegement complete with a personal contact reference at ASX but feedback to date.
