Biv pacing is used to resynchronize LV contraction which increasings cardiac output acutely and in the longterm improves cardiac function. It is only beneficial in pts with LV dyssynchrony which is determined throught the ECG or echo. Some pts have a dramatic improvement in functional status even if LV function only improves modestly. The cost is upt to 50,000 if an ICD (defibrillator) is used. Devices like this and medical therpay (dilatrend, ACEI) are decreasing the need for transplant or destination VAD's.
