I am currently in the midst of my annual course of pentosan (with some complicating factors this time that I will talk about elsewhere). My doctor now prescribes me an 8 week course of 16 injections. I think this longer course is a good idea. From my reading of the data it looks to me like the drug effect is not fully saturated at the 6 week mark. We do have non-responders to the treatment and I wonder if some of those might become "responders" under an extended course. I don't think there's any risk and if it were me I'd be running the longer course.
I definitely did find a bit of irritation at the injection site so you are not alone. It was worse the first two years I did it. I haven't really noticed it this time around. It's a minor annoyance, and one I would take over arthritis any day of the week. I have never experienced any lethargy or tiredness.
I didn't feel any improvements for the first three weeks. Started to notice improvement in week 4. But then it just kept building week by week. The studies show effect peaks around 56 days and that was consistent with my experience. So be patient.
