Probably an off market transfer into your CMC holding. Form will have to be signed by both named persons.

I transferred my own issuer sponsored WBC shares into my CMC holding this way. Fill out and send in to CMC, they'll handle it. should cost like $50 bucks.
Word to the wise, carefully consider what you report as the "consideration" for the shares, as it may trigger CGT event for one or more of the original named holders.
