I only do this as a past time and you are obviously a seasoned trader, so I'm pretty sure I'm not telling you anything but...
I've done off market transfers myself, there's no fees involved whatsoever, so why wouldn't you avoid paying a percentage of a large transaction if you could. I doubt whether it would appear on the 'market' though. This could be the case of somebody transferring from their personal account to a superannuation account, family trust or something, or vise versa as I've seen Directors do on occasion.
I'm assuming that if it was the case that somebody was exercising an option/derivative then there would be a relevant notation on the 'condition' field.
