Are you talking transmission or distribution?
Distribution lines up to 22,000V are run under ground these days in all new subdivisions - they are cost effective.
When you step up to transmission lines - 100,000V and above, that's where the insulation requirements become much more critical.
High voltage current is a beast and it needs proper containment.
That containment is not cheap.
