This Harvard Medical School study was funded by the Agency for Healthcare Research and Quality, an agency within the US government Department of Health and Human Services, and found that “fewer than 1% of vaccine adverse events are reported” so the study was ignored and shelved because it did not fit the agenda of health authorities to maximise vaccine uptake by promoting public confidence in their vaccination programs.

So according to the Harvard Medical School study, using VAERS reports to 30 April, at least 15,727,700 Americans experienced adverse events following covid vaccination, 1,601,400 experienced serious injuries and no fewer than 383,700 Americans died following covid vaccination.

No wonder health authorities don't want to develop an accurate vaccine adverse event reporting system.
