CPT Code

Does anybody know whether AXP has to apply to the US Centre for Medicare and Medicaid Services for a CPT category description and valuation??

This category represents the rates of reimbursement paid to practitioners for the relevant medical procedure by Medicare. Two other companies I know about recently applied for (via the relevant medical association) and received this code; ACG and IPD. Does it apply for breast reconstruction following mastectomy??
