Fair comments about the third world market position, however I think Bill Gates and others do get the point. Opportunity may not yet exist for treatment/erradication with the "tribal masses" quite at this point in time but we are seeing significant momentum building.
There would still sooner of later seem to be a need for screening for hospitals, places of learning etc in the cities and regional centres where development is occurring.
