I was extremely disturbed to receive my Registration Forms with my old address
of 3 and a half years ago on the documents. Fortunately, it was redirected as seen
on a copy of your envelope, above.
An Annual Report also just received was sent to my active address.
My concern is this:
a. What other correspondence was sent to old address and may have been lost?
b. What administrative oversight allowed this to happen in my case?
c. What steps will your Management undertake to bring absolute certainty to the security of member records?
In the meantime, I request that you conduct a thorough search of my sent-mail register and:
1. identify mail that may have been sent to OLD ADDRESS;
2. resend me copies of the mail so identified to my NEW ADDRESS.
This will be much appreciated.
