With the amount of money the VA have been allocated, I am sure they will seek to address their demands via every possible route. 4D scans may be better in every way, but the fact is there aren't defined techniques and procedures developed for using them to diagnose conditions that have been taught to professionals through their university studies and placements. There aren't endless case studies and published trials available to be studied. There are qualified, experienced nuclear medicine experts, tried and tested procedures, etc etc. for nuclear medicine. Of course, they can't say no to spending $$ via the existing proven method for treating the conditions in question.

Just have to hope we will be hot on their heals being used in parallel so that eventually the better diagnosis method - less intrusive, faster, cheaper, healthier etc etc. can take over from the existing method. Everybody seems to underestimate how slow moving and siloed medical institutions are, every institute will need to develop their own proof, procedures, documentation, training, etc, etc. But many of the biggest and most famous medical institutions in the world already started doing this years ago, the wheels are turning, momentum is building, and an incredible moat is slowly being built around our technology, but dont expect some new institutions to sign contracts to start scanning bulk patients overnight.
