I think I generally get greedy when watching the charts for too long without break with an order ready to go. I"ll start to think "If I entered there I would already have 4 ticks profit" and then it continues to move in the correct direction and that split second impulse kicks in and before I realise it, I enter the trade. That slow build up of angst as the candles keep moving amazingly and you missed your entry. Only happens when I trade for too long.
