A ballot doesn't need to be physically damaged for it to be damaged. The ink could be faint where it isn't read by a computer, the QR code ink on the paper could be thin. Could be a million reasons.
But that is the answer and that is what is going on here. You can take that or you can wilfully pretend its something else.
