Contaminated Endoscopes Suspected in CRE Outbreak in California

Seven patients at UCLA's Ronald Reagan Medical Center have been infected with carbapenem-resistant Enterobacteriaceae (CRE), two of whom have died, the Los Angeles Times reports.

The infections appear to be linked to contaminated duodenoscopes. Even with meticulous cleaning, bacteria can persist on the devices.

UCLA has notified nearly 180 patients that they may have been exposed to CRE from October to January during endoscopic retrograde cholangiopancreatography (ERCP). After conducting a safety visit, county officials said there were no infection control breaches. Nonetheless, the hospital switched to gas sterilization after it learned of the outbreak.

The FDA says it received reports of 135 patients in the U.S. becoming ill from potential microbial transmission from reprocessed duodenoscopes from 2013 through 2014. Facilities should strictly follow manufacturers' instructions for reprocessing to minimize the risk to patients, the agency says.

Patients should be advised to call their physician if they experience the following symptoms after ERCP: fever or chills, chest pain, severe abdominal pain, difficulty swallowing or breathing, nausea, vomiting, or black or tarry stools.
