easiest way to find historical PEN cap raises and option raises / conversions

Hi PEN'ers

what is the easiest way to find historical PEN cap raises and option raises / conversions from the past?

for example:
if you need to look up if you participated in a PEN cap raise or options (PENOC or PENOD) cap raise?
if you converted PENOC or PENOD into PEN where can you look this up?
and has there only been one consolidation (40:1)?
