Rorter your report on this accident is spot on,as part of my job I have to perform concrete pump checklists on any concrete pumping activities onsite which also involves records of pipe thicknesses from the piping used on the pumps they are crazed at least twice a year so the wall thicknesses are monitored and pipes can be changed out when they get to thin in the walls to mitigate risks of pipes blowing out and shooting concrete everywhere which can be very dangerous. These pipes can explode it’s happened a few times that I know of and have perform a few toolbox talks on this topic . Not only the walls of the pipes but the joints can let go to there are controls in place which lowers the risk of it happening but the consequences if something went wrong are still dangerous
