Lots of incompetence and cost blow-outs in the corporate world in general. Cost blowouts easier to deal with in huge corporations where they can be hidden, rather than in mining explorers transitioning to producers. Management invariably have big ego's and are 'game players' who clambered over the bodies of others to get to the top, but are not necessarily deep thinkers. Also contrary positions - often 'the truth' - are seen as 'negative', everyone saying 'yes - we agree!' is the way to win approval and career advancement.
