Generally it is a fault in one of the thousands of lithium ion cells in the big battery which gets hot and then starts a gradual fire inside the battery which eventually explodes into an inferno releasing clouds of poisonous gas.
All it can take to make one of these Lithium Ion fire bomb batteries explode is a severe bump like in an accident. Lord help the driver if the silly doors jamb!!!
