Each document has a unique reference number, and multiple acceptances must be paid separately.  If you have shares in more than one account you need to download the form for each account.

You can pay by BPAY or by EFT, but not by cheque or money order.

From the form: You do not need to return this form if you have made payment via BPAY®. Your BPAY® reference number will process your payment to your entitlement electronically and you will be deemed to have applied for such New Shares for which you have paid.

The form needs to be returned if you choose to pay by EFT.

Allow time for your payment to be processed before the closing time of 5pm on 11 June.
