As for this:

Simply don't agree with this but it's nice theoretical waffle. Every situation needs to be evaluated on its merits. There are many bad things that happen within organisations everyday but I don't prescribe to this theory that the CEO takes the blame for every event. Certainly isn't my experience. Typically, an event will be evaluated, action taken, and preventtive measures devised to prevent it happening again. There's always a scapegoat but rarely unless very very serious will it be the CEO.
