1. Probably a small bank fee or direct debit set up fee along the way. I remember getting one like that but for $2 did not bother following up.
2. ATO levy has gone up and is usually paid as part of annual tax return
3. I have found service fine but have never needed to phone them. I would expect better service from an accountant at 5 times the price, but am a cheapskate so esuperfund suits me
