I contacted the Company Secretary about a month ago & she emailed me the form. You just need to fill it in & return it with your cheque to Computershare in Perth before the expiration. The Company Secretary is Sarah Smith and her email is [email protected]
It seems that 88E "outsource" a lot of the Company Secretarial Matters to Mirador Corporate in Perth & I would suspect that Sarah is pretty busy. Therefore I would suggest that you contact Computershare directly.
I am waiting until about mid January to do anything with mine.
